•Use this portal for medical certificate submissions only! Any merchant mariner credential (MMC) applications or awaiting information documentation submitted through this portal will NOT be processed.
•Combine your .pdfs into a single submission. Do Not submit individual pages for the CG-719K (or CG-719K/E) form separately. The form should be combined into a single .pdf attachment.
•Maximum file size for each upload is 75mb.
•Do not password protect your documents. We can’t open them and they will not be processed.
