​Portal Instructions:
       1) All pages of medical certificate application MUST be included and can only be ONE file.
       2) PDF uploads must be titled LASTNAME_FIRSTNAME_Ref#. If No reference number, LASTNAME_FIRSTNAME. Ex SMITH_JOSH_5555555 / SMITH_JOSH.
