• First Class Pilot and Local Limited applicants ONLY:
• DO NOT use ASAP to apply. Please submit your application via e-mail to the Regional Exam Center that services the requested routes.

• All other Merchant Mariner Credential applicants:
• Use this portal for merchant mariner credential (MMC) submissions only! Any medical certificate applications or awaiting information documentation submitted through this portal will NOT be processed.
• Combine your .pdfs into a single submission. Do Not submit individual pages for the CG-719B form separately. The form should be combined into a single .pdf attachment. Other multi-page documents should be combined into a single .pdf attachment as well.
• Maximum file size for each upload is 75mb.
• Do not password protect your documents. We can’t open them and they will not be processed.
