IMPORTANT – READ FIRST BEFORE SUBMITTING ANY DOCUMENTATION!
•Use this portal for merchant mariner credential (MMC) submissions only! Any medical certificate applications or awaiting information documentation submitted through this portal will NOT be processed.
•Combine your .pdfs into a single submission. Do Not submit individual pages for the CG-719B form separately. The form should be combined into a single .pdf attachment. Other multi-page documents should be combined into a single .pdf attachment as well.
•Maximum file size for each upload is 75mb.
•Do not password protect your documents. We can’t open them and they will not be processed.
